Evelyn Elaine Bolen, 17, of 2611 S. CR 550W, Warsaw, died Sept. 3, 2008, in Lutheran Hospital, Fort Wayne.
She was born Feb. 15, 1991, in Woodlawn Hospital, Rochester, to Delbert and Norma Isley Bolen.
She was a junior at Tippecanoe Valley High School, Akron, and a member of Amazing Grace Community Church, Sidney.
Also surviving are four sisters: Peggy (and spouse Mark) Davis, Warsaw; Norma (and spouse Lonny) Hodges and April (and spouse Michael) Meade, all of Mentone; and Regina Bolen, Claypool; and a brother, Noble Bolen, Warsaw.
Services will be at 1 p.m. Monday at Redpath-Fruth Funeral Home, Warsaw, with pastors Ballard Wood and Don Arms officiating. Burial will be in Palestine Cemetery.
Calling is from 2 to 8 p.m. Sunday and from 10 a.m. to 1 p.m. Monday at the funeral home.
