NOTICE OF DISSOLUTION OF SOMMERS VETERINARY CLINIC, INC.
Notice is hereby given that SOMMERS VETERINARY CLINIC, INC., an Indiana corporation, has been dissolved. Any person/entity with any claim against the corporation must present such claim as follows. Each claim must include: 1) the asserted amount of the claim; 2) the date incurred; 3) sufficient information giving the basis for the claim; 4) the full name, address, telephone/fax numbers and e-mail address of the contact person presenting the claim.

A claim against the company will be barred unless a proceeding to enforce the claim is commenced not more than two (2) years after the publication of this notice.
