Creassia Blackburn, 48, Mentone, died Sept. 16, 2026.
She was born on Nov. 3, 1977, to Davey and Sharon Meade Perkins in Warsaw. She married Walter Blackburn on Jan. 13, 2001; he survives.
Titus Funeral Home and Cremation Services of Warsaw is entrusted with Creassia's care. Her Life Celebration visitation will be held on Tuesday, Sept. 22 from 3 p.m. to 7 p.m. at Pentecostal Lighthouse Church, Warsaw. A church service will follow that evening, starting at 7 p.m. at the church. An additional visitation will be held on Wednesday, Sept. 23 from 10 a.m. to 11 a.m. at Pentecostal Lighthouse Church, followed by the funeral service starting at 11 a.m. with Pastors Samuel Perkins and Glen Hughes officiating. Her burial will follow the service at 2791 W. CR 950S, Silver Lake.
