Viking Dr., Mentone, IN 46539 and if different, my residence address is: in the Kosciusko County, Indiana, hereby gives notice that Colleen Shafer has filed a petition in the Kosciusko Court requesting that her name be changed to Colleen Diane Craig.
Notice is further given that hearing will be held on said Petition on the December 11, 2020 at 8:15 a.m.
