I, Molli Baez, whose mailing address is: 10005 N Price Rd., Milford, IN 46542. And, if different, my residence address is: in the Kosciusko County, Indiana hereby gives notice that Molli Baez has filed a petition in the Circuit Court requesting that her name be changed to Milli Rico.
Notice is further given that hearing will be held on said Petition on the November 1st, 2019 @ 8:30 am.
