Comes Now Petitioner, Annalyse Lindsey, and provides notice as follows:
1. Annalyse Lindsey has filed a petition to change the name of minor child in Kosciusko Circuit Court under cause number 43C01-2010-MI-107.
2. Blake Lewis Watters is the name of the minor child for whom petition has been filed.
3. The new name requested is Blake Lewis Lindsey.
4. This action is pending in the Kosciusko Circuit and the petition was filed on October 14, 2020. Hearing on this matter will be had in the Kosciusko Circuit Court on December 11, 2020 at 9:00 a.m.
5. Any person has the right to appear at the hearing and or to file an objection. Wherefore, Petitioner files notice by publication pursuant to IC 34-28-2-3.
