Notice is hereby given that Dakota Ray Lance Conway has applied to the Kosciusko Court, Kosciusko County, Indiana, for a change of the Petitioner's name to Dakota Ray Clark, and that said petition for change of name has been set for hearing before the Court on the 4th day of December, 2020, at 9:30 a.m.
