MADISON - Chad William Phillips, 34, of Commiskey, near Lancaster, died at 10:30 p.m. Sept. 15, 2009, at King's Daughters' Hospital, Madison, Ind.

He was born Aug. 22, 1975, in Madison, to Larry William and Deborah Jean "Debbie" Walker Phillips, who survive in Lancaster.

He resided all of his life in the Lancaster community and was a 1993 graduate of Madison Consolidated High School. He was a member of Prince of Peace Catholic Church. He enjoyed helping his father on the family farm and going to his father's construction sites with the Larry Phillips Construction Co. On Jan. 2, 1987, he was the first kidney transplant patient at Kosair Children's Hospital, Louisville, Ky. He enjoyed collecting Ford tractors, collecting hats, caring for his animals and loved coon hunting with his father early in life. He loved playing with his dog, Lucy, and his red Ford truck. He had been in ill health for several years and was lovingly and tenderly cared for by his devoted parents. He will be greatly missed.

He will be sadly missed by his loving and devoted parents; his loving maternal grandfather, Walter Walker, and his companion, Norma Farrar, Lancaster; his loving paternal grandfather, Morris L. Phillips, Lancaster; his loving aunts and uncles, Shirley and Jerry Clevenger, Winona Lake; Linda and Jim Botkins, Versailles, Ky.; and Lowell Phillips and Kelley Phillips, both of Lancaster; his loving cousins: Kady and Tyson Becker; Joseph and Gina Clevenger; Jonathan and Karley Sasso; and Jonathan Phillips; his devoted best friend: Timmy Gayle, Madison; and several great-aunts, great-uncles, other cousins and other relatives. He was preceded in death by his maternal grandmother, Golda Colber Walker, who died Feb. 16, 2000; and his paternal grandmothers: Polly Eversole Phillips and Gladys Halcomb Phillips, who died Aug. 7, 2009.

Services will be at 10:30 a.m. Tuesday at Prince of Peace Catholic Church, 413 E. Second St., Madison, with the Rev. John A. Meyer officiating. Burial will be at a later date in Indiana Veterans Memorial Cemetery, Madison.

Calling is from 5 to 8 p.m. Monday and from 9:30 to 10:10 a.m. Tuesday at the Morgan & Nay Funeral Centre, 325 Demaree Drive, Madison.
