Carol Jean Slack Smith, 48, of Fort Myers, Fla., and formerly of Warsaw, died Feb. 15, 2011, in Health Park Medical Center, Fort Myers.
She was born Oct. 14, 1962, in Madison, to Chauncey and ClaraBelle Slack. In March 1991, in Warsaw, she married Kevin Smith.
Raised in Vevay, she was a 1980 graduate of Switzerland County High School and received a bachelor's degree in interior design in 1984 from Indiana State University in 1984. She began her career in interior design in 1985 at Compton Furniture, Warsaw, before moving to Florida where she continued her design career.
Surviving are a daughter, Candace, Fort Myers; a son, Kameron, Fort Myers; her father, Chauncey Slack, Pleasant; a sister, Kathy May, Madison; a brother, Curtis Slack, Madison; many nieces and nephews; and her mother- and father-in-law, Jim and Kay Smith, Warsaw. She was preceded in death by her mother, ClaraBelle Slack.
Services were Feb. 21 in Vevay.
