Banner Medical Warsaw located at 1295 Polk Drive, Warsaw, IN 46580, is submitting an amended NOI for this facility to notify the Indiana Department of Environmental Management of our intent to comply with the requirements under 327 IAC 15-6 to discharge storm water exposed to industrial activities. Run-off from the facility will discharge to a retention pond just South of the facility’s property as well as into an unnamed ditch that discharges into the Tippecanoe River.
