Jeffery “Jeff” Moore, 55, Warsaw, died Oct. 17, 2024, at Parkview Regional.
He was born Feb. 8, 1969, in Fort Wayne, to Phillip Moore and Cheryl Fordyce. He married Lynn Thomas.
Titus Funeral Home and Cremation Services is entrusted with Jeff’s care. Visitation for Jeffery will Nov. 7 from 5 to 7 p.m. at the funeral home, Warsaw, with his Life Celebration immediately following.
In lieu of flowers, donations may be given to Turning Point Counseling Center, 501 W. Washington St., North Webster, IN 46555.
