Community Health Systems (4000 Meridian Blvd. Franklin, TN 37067) is submitting a Notice of Intent to the Indiana Department of Environmental Management of our intent to comply with the requirements of 327 IAC 15-5 to discharge stormwater from construction activities associated with the LKH OR Renovation and Lobby Addition located at 2101 East Dubois Dr. Warsaw, IN 46580. Runoff from the project site will discharge to Beyer-Brady Ditch and Pike Lake.
