• 10:33 a.m. Wednesday - Ali Tareq Smith, 21, Fort Wayne, arrested for operating a vehicle while intoxicated. Bond: $700.

• 12:22 p.m. Wednesday - Lavon Tevell Thomas, 46, Chicago, arrested for misdemeanor failure to appear. Bond: $400.

SUV struck NIPSCO utility pole, causing moderate damage to pole and electrical components, causing a power outage. Driver: Larry D. Tackett, 14, South Columbia Street, Warsaw. Injuries: Tackett complained of abrasions to left side of his jaw and neck; passenger Averie Neuwirth, 13, Highlander Drive, Warsaw, complained of abrasions to her chest. Damage: $10,000-$25,000. • 7:24 a.m. Wednesday - West CR 200S, 100 feet east of South Julian Drive, Warsaw. SUV-pickup collision. Drivers: Darla L. Adams, 50, West Main Street, Mentone; Jerry A. Lane, 52, Francesville. Damage: $2,500-$5,000. Kosciusko County Sheriff’s Office investigated.

• 8:57 a.m. Tuesday - Theft of an Indiana license plate was reported stolen in the 11800 block of North Robin Hood Road, Cromwell. KCSO.
