• 4:04 p.m. Monday - De Kota Allen Holbrook, 24, Fort Wayne arrested for misdemeanor failure to appear, felony failure to appear, false informing and two charges of theft. No bond set.

• 5:28 p.m. Monday - Christopher Wane Stern, 43, of 822 Lydia Drive, Warsaw, arrested for felony failure to appear and possession of marijuana/hash oil/hashish. Bond: $5,850.

• 4:11 p.m. Monday - Intersection of East Ind. 14 and South Packerton Road, Claypool. Car-van collision. Drivers: Timothy Wyma, 73, Sanford, Fla.; Ashley N. Harman, 25, South CR 200W, Claypool. Injuries: Harman complained of pain in her chest and legs and was transported to the hospital; a passenger in Harman’s vehicle, Harley Harman, 3, South CR 200W, Warsaw, had an abrasion on his chest. Damage: $10,000-$25,000. Kosciusko County Sheriff’s Office investigated.
