Project:    Reconstruction of Lincoln Drive and Johnson Street

    Notice is hereby given that the Board of Public Works and Safety for the City of Warsaw, Kosciusko County, Indiana will receive sealed bids for the above described “Project” at the office of Public Works, 794 W. Center Street, Warsaw, Indiana until 10:00 EST on or before May 8, 2020, and commencing as soon as practicable thereafter on the same date such bids will be publicly opened and read aloud at the Publics works office.  No late bids will be accepted.

    All bids and proposals shall be properly and completely executed on the proposal forms provided with the plans and specifications, which will include the non-collusion affidavit as required by the State of Indiana.  The bid envelope must be sealed and have the words.

“BID – RECONSTRUCTION OF LINCOLN DRIVE AND JOHNSON STREET”

    A bid bond or certified check in an amount not less than ten percent (10%) of the amount bid must be submitted with each bid.  A one hundred percent (100%) performance, a one hundred percent (100%) payment bond and a ten percent (10%) maintenance bond will also be required of the successful bidder.  It is intended that actual construction of all work divisions shall be started as soon as practicable, and each bidder shall be prepared to enter promptly into a construction contract, furnish a performance bond, and begin work without delay in the event the award is made to him.

    The Project consists of, but is not necessarily limited to, the following:

from Lincoln Street to Lindberg Drive in Warsaw, Indiana

    Contract Documents for the Project have been assembled into one bound project manual, which together with drawings, may be examined at the following location:

    Plans and bid documents may be obtained from USI Consultants, Inc., 8415 E. 56th Street, Indianapolis, IN 46216. A non-returnable fee of $100.00 will be charged for each printed set desired and can be shipped via UPS for an additional $50.00 shipping fee.  Plans and bid documents can also be provided electronically in PDF format for a non-returnable fee of $25.00.  Checks shall be made payable to USI Consultants, Inc.  Prospective Bidders can contact USI Consultants at 317-522-2479 Attention: Michael Breach or email [email protected] to request plans and bid documents. Bidding documents must be purchased to be eligible to bid on this contract.  The purchase of the Contract Documents is non-refundable.

    Bidders shall assure that they have obtained complete set of plans and bid documents and shall assume the risk of any errors or omissions in bids prepared in reliance on incomplete set plans and bid documents.

    This Project will be funded by the City of Warsaw and a Community Crossings Matching Grant.

    The Contract will be awarded in accordance with the provisions of IC 5-16-13 for Public Works projects awarded after June 30, 2015.  Pursuant to IC 5-16-13-7, the provisions of the law are incorporated by reference.

    In accordance with IC 4-13.6-4, Contractors entering into a contract with the Owner that is public work, and contractors of any tier as defined in IC 5-16-13-4, will be required to be qualified by the Indiana Department of Administration’s certification board.

    In accordance with IC 8-23-10, Contractors entering into a contract with the Owner, and contractors of any tier as defined in IC 5-16-13-4, working on a project that is the construction, improvement, alteration, repair or maintenance of a road, highway, street, or alley will be required to be qualified by the Indiana Department of Transportation.

    In accordance with IC 22-5-1.7-11.1, Contractors entering into a contract with the Owner, and contractors of any tier as defined in IC 5-16-13-4, will be required to enroll in and verify the work eligibility status of all newly hired employees through the E-Verify program (effective July 1, 2011), and comply with the reporting requirements of IC 5-16-13-11.

    In accordance with IC 5-22-16.5, Contractors entering into a contract with the Owner will be required to provide an Indiana Iran Investment Certification.

    In accordance with IC 5-16-13-11, Contractor shall submit an Employee Drug Testing Plan with their Bid and contractors of any tier as defined in IC 5-16-13-4 must comply with the drug testing requirements set forth in IC 4-13-18.

    The City of Warsaw in accordance with Title VI of the Civil Rights Act of 1964, 78 Stat. 252, 42 U.S.C 2000d to 2000d-4 and Title 49, Code of Federal Regulations, Department of Transportation, Subtitle A, Office the Secretary, Part 21, Nondiscrimination in Federally assisted programs of the Department of Transportation and Title 23 Code of Federal Regulations, Part 200, Title VI Program and Related Statutes, issued pursuant to such Acts, hereby notifies all bidders that it will affirmatively insure that in any contract entered into pursuant to this advertisement, disadvantaged business enterprises will be afforded full opportunity to submit bids in response to this invitation and will not be discriminated against on the grounds of religion, race, color, national origin, sex, sexual orientation, gender identity, age, disability/handicap and low income in consideration for an award.

    Anyone who requires an auxiliary aid or service for effective communication, or a modification of policies or procedures to participate in a program, service, or activity of the City of Warsaw, should contact the office of Jeremy Skinner, 102 S.     No bidder may withdraw any bid or proposal within a period of 30 days following the date set for receiving bids or proposals. The Warsaw Board of Public Works and Safety reserves the right to hold any or all bids or proposals for a period of not more than 30 days and said bids or proposal shall remain in full force and effect during said period.  The City of Warsaw reserves the right to reject and/or cancel any and all bids, solicitations and/or offers in whole or in part as specified in the solicitations when it is not in the best interests of the governmental body as determined by the purchasing agency in accordance with IC 5-22-18-2
