• 12:26 a.m. Monday - Wyatt D. Extrom, 19, of 483 S. CR 800W, Warsaw, arrested for operating a motor vehicle while intoxicated. Bond: $700.

• 12:53 a.m. Monday - William Harvey Epperly III, 61, Tipton, arrested on a fugitive warrant. Bond: $50,000.

• 7:02 a.m. Monday - Sylvia M. Perales, 56, of 530 S. First St., Pierceton, arrested for check deception. Bond: $191.81.

• 9:53 a.m. Monday - Douglas Charles Jones, 50, of 612 E. Lakewood Ave., Warsaw, arrested for leaving the scene of a crash with property damage. Bond: $500.

• 12:14 p.m. Monday - John Bradly Wood, 24, Bristol, arrested for failure to appear - misdemeanor. Bond: $476.45.

• 12:32 p.m. Monday - Jeremy Allen Bogner, 30, Columbia City, arrested for failure to appear - misdemeanor. Bond: $250.98.

• 4:21 p.m. Monday - Tasha Marie Bratten, 29, Elkhart, arrested for failure to appear - misdemeanor. Bond: $285.

• 4:53 p.m. Monday - Patrick Ryan Flick, 29, Elkhart, arrested for failure to appear - misdemeanor. Bond: $275.

• 6:51 p.m. Monday - Preston Michael Gibson, 30, of 303 E. John St., Syracuse, arrested for possession of methamphetamine and driving while suspended with a prior suspension within the last five years. Bond: $5,250.

• 9:04 p.m. Monday - Jordan Todd Burgess, 27, of 202 S. Morgan St., Mentone, arrested for criminal mischief. Bond: $500.

• 11:45 p.m. Monday - Debbie Sue Dotson, 56, of 14533 N. CR 300W, Silver Lake, arrested for operating a motor vehicle while intoxicated. Bond: $700.

• 1:52 a.m. Tuesday - Matthew A. Klosowski, 52, of 1105 E. Wellington Drive, Warsaw, arrested for operating a motor vehicle while intoxicated. Bond: $700.

• 3:42 a.m. Tuesday - Melissa Yvonne Coy, 40, of 600 N. Colfax St., Warsaw, arrested for operating a motor vehicle while intoxicated and operating a motor vehicle while intoxicated endangering the lives of others. Bond: $700.

• 2:47 p.m. Tuesday - Paul Dean McCarthy, 38, of 909 Sara St., South Whitley, arrested for failure to appear. Bond: $99.

• 11:45 p.m. Tuesday - Jeremiah Jameslee Moore, 39, of 1341 Gable Drive, Warsaw, arrested for possession of methamphetamine, possession of a hypodermic syringe or needle and on a warrant. Bond: $5,250.

Personal Injury Accident

• 9:10 p.m. Monday - 7822 E. Epworth Forest Road, North Webster. SUV hit utility pole. Driver: Santiago C. Gonzales, 32, South Kinder Drive, Syracuse. Injury: Gonzales complained of chest pain but refused medical attention at the scene. Damage: $2,500-$5,000. North Webster Police Department investigated.



Property Damage Accidents

• 12:29 p.m. Monday - West U.S. 30, 1,200 feet east of North CR 200W, Warsaw. Van went off road and into a ditch. Driver: Abdullah Mbai, 54, Reynoldburg, Ohio. Damage: $1,000-$2,500. Warsaw Police Department investigated.

• 7:29 p.m. Monday - Ryerson Road, 700 feet west of CR 900E, Pierceton. Pickup hit deer. Driver: Kyle A. Reed, 16, West Old Trail Road, Pierceton. Damage: $1,000-$2,500. Pierceton Police Department investigated.

• 5:52 p.m. Tuesday - East CR 800S, 100 feet east of South CR 300E, Claypool. Pickup hit parked pickup with trailer registered to Roger V. Rice, Huntertown. Driver: James C. Shuster, 25, East CR 800S, Claypool. Damage: $5,000-$10,000. Kosciusko County Sheriff’s Department investigated.

• 3:06 p.m. Friday - Criminal mischief was reported in the 500 block of North Front Street, Syracuse. Syracuse Police Department investigated.

• 6:16 p.m. Sunday - A traffic stop occurred in the 1500 block of West U.S. 30, Warsaw. WPD.

• 6:47 p.m. Sunday - A battery with serious bodily injury was reported in the 1700 block of West Lake Street, Warsaw. WPD.

• 7:07 p.m. Sunday - Theft of a cell phone was reported in the 2400 block of East Center Street, Warsaw. Value: $600. WPD.

• 11:39 p.m. Sunday - A warrant was served in the 200 block of North Detroit Street, Warsaw. WPD.

• 1:49 p.m. Monday - A lost or stolen license plate was reported in the 200 block of West Main Street, Warsaw. KCSD.

• 8:32 p.m. Monday - A warrant was served in the 200 block of North Lake Street, Warsaw. WPD.

• 9:25 p.m. Monday - A person was arrested in the 600 block of West CR 700S, Claypool, for operating a motor vehicle while intoxicated. Silver Lake Police Department investigated.

• 11:40 p.m. Monday - A possible suicidal person was reported in the 500 block of West Main Street, Mentone, and the investigation led to the discovery of a violation of a no-contact order. KCSD.

• 2:39 a.m. Tuesday - A woman was arrested in the 600 block of North Colfax Street, Warsaw, for operating a motor vehicle while intoxicated. WPD.

• 6:34 p.m. Tuesday - A suicidal person was reported in the 700 block of North Detroit Street, Warsaw. WPD.
