• 10:33 a.m. Monday – Sidney R. Ousley, 18, Rochester, arrested for misdemeanor failure to appear. Bond: $400.

• 2:07 p.m. Monday – Justin D. Birchfield, 29, of 1386 E. Dove Lane, Milford, arrested for theft. Bond: $600.

• 2:30 p.m. Monday – George R. Adams II, 48, Bremen, arrested for leaving the scene of an accident. Bond: $500.

• 3:53 p.m. Monday – Adam E. Hurley, 39, of 502 N. Mill St., North Manchester, arrested for misdemeanor probation violation. No bond set.

• 11:38 p.m. Monday – Mark A. Holbrook II, 41, of 920 Anchorage Road, #11, Warsaw, arrested for driving while suspended with a prior suspension. No bond listed.

• 11:43 p.m. Monday – Stacy A. Holbrook, 44, of 2605 10B Road, Bourbon, arrested for possession of methamphetamine. Bond: $5,250.

• 9:29 a.m. Monday – Hand Avenue, 500 feet north of West Winona Avenue, Warsaw. Box truck hit top of bridge, got stuck. Driver: Timothy W. Lee, 32, North Ind. 5, Larwill. Damage: $5,000-$10,000. Warsaw Police Department investigated.

• 2:39 p.m. Monday – Intersection of East Center and South Roosevelt Streets, Warsaw. Van-SUV collision. Drivers: Linda K. Norman, 70, South Pelican Way, Claypool; Madelyn N. Nowak, 18, East Main Street, Warsaw. Damage: $1,000-$2,500. WPD.

• 4:56 p.m. Monday – South McKinley Street, 100 feet north of East Winona Avenue, Warsaw. Car-van collision. Drivers: Brandi L. Harvey, 19, Avilla; Nancy L. Corl, 62, Country Club Drive East, Warsaw. Damage: $5,000-$10,000. WPD.
