Williams, 46, North Fox Farm Road, Warsaw; Taylar C. Roberts, 27, East Smith Street, Milford. Injury: Ezekiel D. Fitzpatrick, 24, East Center Street, Warsaw, a passenger in Ortiz’s vehicle, complained of head pain. Damage: $50,000-$100,000. Kosciusko County Sheriff’s Office investigated.

Car went into Lake City Highway median, hit an Indiana Department of Transportation sign, drove out of the median, hit rear of semi-tractor trailer, went down into a ditch and came to a rest against a chain-link fence. Drivers: Elisabeth A. Hoffman, 24, East Fort Wayne Street, Warsaw; Norman L. Ullery, 46, North Main Street, Bourbon. Damage: $5,000-$10,000. Warsaw Police Department investigated.

• 3:50 p.m. Tuesday - Intersection of South Buffalo Street and Fisher Avenue, Warsaw. Car-SUV collision. Drivers: Esther R. Osborne, 38, Oriole Lane, Warsaw; Jennifer D. Shepherd, 44, Columbia City. Damage: $1,000-$2,500. WPD.

• 10:14 p.m. Tuesday - 2519 E. Center St., Warsaw. Commercial truck hit overhang of Wyndham Gardens, East Center Street, Warsaw. Driver: Christopher L. Carson, 51, Tracy City, Tenn. Damage: $10,000-$25,000. WPD.

• 7:06 a.m. Wednesday - Intersection of South Ind. 15 and Herscher Drive, Warsaw. Car-SUV collision. Drivers: Micah L. Hoffman, 18, Gable Drive, Warsaw; Mark G. Fick, 50, EMS T6A Lane, Leesburg. Damage: $1,000-$2,500. WPD.

• 3:24 p.m. Tuesday - A man reported his employer came to him about threats that were made in the 6500 block of West Crystal Lake Road, Warsaw. KCSO.

• 5:19 p.m. Tuesday - Criminal mischief was reported in the 3400 block of West Warren Drive, Warsaw. A woman reported a vehicle window was broken out. Damage: $200. KCSO.
