• 10:40 a.m. Monday - Anton Tony Sullivan, 58, of 521 N. Colfax St., Warsaw, arrested for failure to appear - original charge misdemeanor. Bond: $292.

• 11:18 a.m. Monday - David Lee Mefford, 20, Fort Wayne, arrested for failure to appear - original charge felony. No bond set.

• 12:43 p.m. Monday - Trentin Dean Garwood, 26, of 101 E. Line St., South Whitley, arrested for failure to appear - original charge misdemeanor. Bond: $5,000.

• 1:06 p.m. Monday - Andrew Tyler Corn, 22, of 208 S. James St., Milford, arrested for criminal conversion. Bond: $600.

• 1:13 p.m. Monday - Jay Anthony Mote, 41, of 1500 Monroe St., Rochester, arrested on a warrant in a civil matter. Bond: $500.

• 5:10 p.m. Monday - Joshua Wayne Alair Galvin, 38, of 920 Anchorage Road, #101, Warsaw, arrested for driving while suspended with a prior and theft with a prior. Bond: $400.

• 6:41 p.m. Monday - Titus Tyrerse Mundy, 19, Inkster, Mich., arrested for failure to appear - original charge felony. No bond set.

Personal Injury Accident

• 2:36 pm. Saturday - Intersection of North Barbee Road and EMS B61 Lane, Warsaw. Tree broke due to high winds and fell on top of SUV and damaged several NIPSCO utility poles and power lines. Driver: Robert L. Johnson, 28, Bridgeview, Ill. Injuries: Johnson complained of pain to his back; a passenger, Stephanie A. Johnson, 30, Bridgeview, complained of pain to her neck; both were taken to the hospital for treatment. Damage: $5,000-$10,000. Kosciusko County Sheriff’s Department investigated.

• 10 p.m. Saturday - Ind. 13, 100 feet north of North Southshore Drive, Syracuse. Car hit deer. Driver: Elizabeth M. Kennedy, 30, Huntington. Damage: $1,000-$2,500. SPD.

• 4:27 a.m. Sunday - East U.S. 30, 1,000 feet east of South CR 900E, Pierceton. SUV hit deer. Driver: Heather L. Scherer, 40, EMS B6A Lane, Leesburg. Damage: $5,000-$10,000. KCSD.

• 1:44 a.m. Sunday - A welfare check was performed in the 800 block of East Hawthorn Drive, Warsaw. KCSD.

• 11:09 a.m. Sunday - A man in the 1600 block of Tippecanoe Drive, Warsaw, reported damage to his vehicle. Damage: $1,500. WPD.

• 11:16 a.m. Monday - An employee of Friendly Express in the 300 block of West Emeline Street, Milford, reported a man causing problems after previously trespassing on the property on Oct. 11. Milford Police Department investigated.

• 12:17 p.m. Monday - An employee of Theresa’s Delectables in the 200 block of North Higbee Street, Milford, reported a man stole a screwdriver and a cordless power saw from contractors working on the shop as he left. Value: $150. MPD.

• 12:43 p.m. Monday - A man reported the possible theft of property in the 4900 block of North CR 250E, Warsaw. KCSD.

• 4:19 p.m. Monday - A person was arrested on a warrant in the 900 block of Anchorage Road, Warsaw. WPD.
