The following people were preliminarily charged and booked into the Kosciusko County Jail:

• 10:52 a.m. Wednesday - Antonio Palacio Fuentes Jr., 52, of 9541 E. Pixie Parkway, Cromwell, arrested on a warrant in a civil matter. Bond: $500.

• 3:56 p.m. Wednesday - Jacob Dale Hamilton, 28, of 135 E. South St., Milford, arrested on five counts of failure to pay. Bond: $1,671.50.

• 4:25 p.m. Wednesday - Amanda Leann Boone, 22, of 1310 E. Clark St., Warsaw, arrested for driving while suspended with a prior. Bond: $450.

• 8:21 p.m. Wednesday - Synethea Shelton Dejhine, 23, South Bend, arrested for possession of meth, possession of paraphernalia - misdemeanor, forgery, identity deception and false informing. Bond: $5,250.

• 9:05 p.m. Wednesday - Tanner Ryan Manns, 22, of 1400 N. CR 875W, Etna Green, arrested for failure to appear - misdemeanor. Bond: $672.70.

• 11:49 p.m. Wednesday - Taylor Ann Lemaster, 26, of 929 E. Sheridan St., Warsaw, arrested for theft, possession of meth and possession of a syringe for a legend drug. Bond: $5,250.

• 11:51 p.m. Wednesday - Jaimie Nicole Albert, 28, of 2176 N. CR 875W, Etna Green, arrested for theft, possession of meth and possession of a syringe for a legend drug. Bond: $5,850.



Personal Injury Accident

• 6:54 p.m. Tuesday - West CR 225N, 300 feet east of North CR 950W, Etna Green. Van hit pedestrian, Mikey Haverstock, 5, West CR 225N, Etna Green, that ran into road. Driver: Carolyn J. Kaiser-Rapp, 76, North CR 875W, Etna Green. Injury: Haverstock complained of pain to his head and body and was flown by Lutheran Air to Parkview Hospital in Fort Wayne. Damage: $1,000-$2,500. Kosciusko County Sheriff's Department investigated.



Property Damage Accidents

• 3:54 p.m. Monday - 306 S. Main St., 30 feet south of Huntington Street, North Webster. SUV-car collision. Drivers: Richard J. Rakoczy, 73, Highland; Darla K. Brown, 67, East Madison Street, North Webster. Damage: $1,000-$2,500. North Webster Police Department investigated.

• 3:08 p.m. Tuesday - Intersection of Logan and West Prairie streets, Warsaw. Two-car collision. Drivers: Madeline E. Kosins, 18, South Stonebridge Road, Warsaw; Jessica Monsivais, 17, Marion Avenue, Warsaw. Damage: $2,500-$5,000. Warsaw Police Department investigated.

• 5:37 p.m. Wednesday - Intersection of East Old 30 and U.S. 30, Warsaw. Van-SUV collision. Drivers: Hannah A. Weber, 27, South Ind. 13, Claypool; Jeffrey S. Larson, 36, EMS B25 Lane, Pierceton. Damage: $5,000-$10,000. WPD.

• 6:49 p.m. Wednesday - Ind. 15, 300 feet north of East Fort Wayne Street, Warsaw. Van-car collision. Drivers: Karisma L. Frantz, 17, East CR 200N, Warsaw; Lauren E. Prater, 27, North Lake Street, Warsaw. Damage: $1,000-$2,500. WPD.

• 8:22 p.m. Monday - A woman in the 200 block of North Perry Drive, Milford, reported a violation of a protective order. Milford Police Department investigated.

• 12:49 a.m. Tuesday - During an arrest in the 700 block of South Main Street, North Webster, meth was found on the subject. NWPD.

• 10:22 a.m. Tuesday - An employee of New Market in the 300 block of South Main Street, North Webster, reported the theft of snack cakes and potato chips. NWPD.

• 12:08 p.m. Tuesday - A man in the 100 block of East Esterbrook Drive, North Webster, reported the theft of a phone. NWPD.

• 4:12 p.m. Tuesday - A traffic stop occurred in the 2300 block of East Winona Avenue, Winona Lake. The driver was found to be driving while suspended with a prior. WPD.

• 7:16 p.m. Tuesday - A man reported he believed his brake lines had been tampered with in the 8200 block of East U.S. 30, Pierceton. KCSD.

• 8:14 p.m. Tuesday - A woman reported the theft of an electric guitar and amplifier from a residence in the 200 block of East South Street, North Webster. NWPD.

• 8:36 p.m. Tuesday - A vehicle hitting a pedestrian was reported in the 400 block of Clinic Court, Warsaw. WPD.

• 4:10 p.m. Wednesday - A traffic stop occurred in the 100 block of South Washington Street, Warsaw. The driver was arrested for driving while suspended with a prior. WPD.

• 4:26 p.m. Wednesday -  A woman went to Lakeland Rehabilitation Center in the 500 block of West Fourth Street, Milford, and demanded mail for a deceased resident, made threats and was disorderly. MPD.

• 10:12 p.m. Wednesday - A theft of various merchandise at Walmart in the 2500 block of Walton Boulevard, Warsaw, led to a drug arrest. Merchandise value: $14.88. WPD.
