The following people were preliminarily charged and booked into the Kosciusko County Jail:
• 1:26 p.m. Thursday - Michelle Ann Reavis, 35, of 3699 N. CR 175E, #63, Warsaw, arrested for driving while a habitual traffic violator. No bond listed.
• 3:23 p.m. Thursday - Devin William Murtaugh, 34, of 7750 S. CR 350, South Whitley, arrested for possession of a hypodermic syringe or needle. Bond: $5,250.
• 6:06 p.m. Thursday - Jason Lee Hathaway, 41, of 510 W. Neer St., Silver Lake, arrested for intimidation - extortion/blackmail. Bond: $5,250.
• 9:13 p.m. Thursday - Travis Jack Carlin, 34, of 2282 S. Cost A Plenty Road, Warsaw, arrested for driving while a habitual traffic violator. Bond: $5,250.
• 12:05 a.m. today - Larry Duane Potts, 38, of 1202 Wooster Road, lot 71, Winona Lake, arrested for disorderly conduct. Bond: $500.

Personal Injury Accident
• 2:02 p.m. Wednesday - Intersection of South Ind. 19 and West CR 800S, Mentone. Two-car collision. Drivers: Sidney R. Ousley, 17, North CR 600E, Rochester; Hazel Schultz, 48, South Caldwell Lake Drive, Claypool. Injuries: Ousley had a right leg injury and was flown by Samaritan to Parkview Regional Hospital in Fort Wayne; Schultz complained of neck, chest and left arm pain and was taken to Kosciusko Community Hospital; Daniel Schultz, 27, North CR 500E, Rochester, a passenger in the Schultz car, complained of leg pain and was taken to KCH. Damage: $10,000-$25,000. Kosciusko County Sheriff’s Department investigated.

• 2:05 p.m. Wednesday - 852 N. Parker St., Warsaw. Van-SUV collision. Drivers: Adrienne R. Phelps, 33, East CR 225S, Winona Lake; Charles D. Hunter, 80, EMS B40A Lane, Leesburg. Damage: $2,500-$5,000. Warsaw Police Department investigated.
• 8:44 a.m. Thursday - Intersection of U.S. 30 and East Old 30, Warsaw. Car-van collision. Drivers: Jodi L. Rogers, 38, Decatur; Joan M. Napier, 85, East Ryerson Road, Pierceton. Damage: $2,500-$5,000. WPD.
• 2:18 p.m. Thursday - Intersection of Fisher Avenue and Ranch Road, Warsaw. Unknown vehicle hit car and fled the scene. Driver remaining: Doris O. Rugs, 80, East Baker Street, Warsaw. Damage: $1,000-$2,500. WPD.

• 8:05 a.m. Wednesday - An employee at Warsaw Community Schools in the 800 block of East Smith Street, Warsaw, reported harassing communications. WPD.
• 7:59 p.m. Wednesday - A traffic stop in the 7900 block of East CR 500N, North Webster, led to an investigation of a person driving while suspended. The person was issued a court date. North Webster Police Department investigated.
• 11:43 a.m. Thursday - The theft of money was reported in the 2500 block of Walton Boulevard, Warsaw. Loss: $300. WPD.
• 2:18 p.m. Thursday - A hit-skip accident was reported in the 900 block of Fisher Avenue, Warsaw. WPD.
• 3:09 p.m. Thursday - A person in the 100 block of North Tamarack Street, Warsaw, was arrested for driving while suspended with a prior suspension with the last five years. WPD.
