APPLIED ASPHALT CRACK SEALANT PERFORMED FOR THE
CITY OF WARSAW, INDIANA FOR THE YEAR 2017
The Board of Works and Safety of the City of Warsaw, Kosciusko County, Indiana, on behalf of all City Departments, will receive sealed bids for 2017 Asphalt Crack Sealant on November 29, 2016 until 10:00 a.m. prevailing local time at the Council Chambers; City Hall; 102 South Buffalo St., Warsaw, Indiana 46580.
Sealed bids will be opened at the Council Chambers on November 29, 2016 at 10:05 a.m. prevailing local time at City Hall and either read aloud or made available for viewing to those interested. All bids are for contract year 2017 (January 1, 2017 through December 31, 2017), unless otherwise noted.
Bid packets shall be picked up and signed for at 794 W. Center Street, Warsaw, IN between the hours of 7 am and 4pm. All technical and project questions shall be directed to Jeff Beeler, Public Works Superintendent, via e-mail only at ([email protected]). Questions over the telephone or person-to-person will not be entertained. If an addendum is required it will be issued by e-mail by those whom have signed for a bid packet.
Bids shall be sealed and with the words “2017 Annual Bids for Asphalt Crack Sealant, City of Warsaw” plainly visible on the front. Also included on the front of the bid package shall be the name, address and telephone number of the individual or company submitting the bids.
All forms and certificates are to be completed fully, accurately, and notarized if required. All forms and certificates are provided with the Bid Proposal Package. Your packet must contain the following items:
1. Bid Response Form
2. Indiana State Bid Form 96 Revised
3. E-Verify
4. Bid Bond or Certified Check
All Warsaw city departments may purchase services and/or product from this bid and are to be invoiced individually. Products and services are to be delivered to any point within the corporate limits of the City of Warsaw as designated by the purchaser.
Disputes or questions regarding accepted bids must be submitted in writing to the Public Works Superintendent no later than 5 calendar days following the Board of Works and Safety bid acceptance.
The start date of services, and/or the delivery date for products being provided, shall be within 5 days of a mutually agreed upon date between the provider and the purchaser. Failure to begin work or deliver may void the bid award and/or be cause to claim the forfeiture of the bid surety.
The Contract will be awarded in accordance with the provisions of IC 5-16-13 for Public Works projects awarded after June 30, 2015. Pursuant to IC 5-16-13-7, the provisions of the law are incorporated by reference.
In accordance with IC 22-5-1.7-11.1, Contractors entering into a contract with the Owner, and contractors of any tier as defined in IC 5-16-13-4, will be required to enroll in and verify the work eligibility status of all newly hired employees through the e-Verify program (effective July 1, 2011), and comply with the reporting requirements of IC 5-16-13-11.
In accordance with IC 5-22-16.5, Contractors entering into a contract with the owner will be required to provide an Indiana Iran Investment Certification.
In accordance with IC 5-16-13-11, Contractors shall submit an Employee Drug Testing Plan with their Bid and contractors of any tier as defined in IC 5-16-13-4 must comply with the drug testing requirements set forth in IC 4-13-18. Contractor shall complete a Drug Testing Plan Certification to be submitted with their Bid.
The City of Warsaw in accordance with Title VI of the Civil Rights Act of 1964, 78 Stat. 252, 42 U.S.C 2000d to 2000d-4 and Title 49, Code of Federal Regulations, Department of Transportation, Subtitle A, Office the Secretary, Part 21, Nondiscrimination in Federally assisted programs of the Department of Transportation and Title 23 Code of Federal Regulations, Part 200, Title VI Program and Related Statutes, issued pursuant to such Acts, hereby notifies all bidders that it will affirmatively insure that in any contract entered into pursuant to this advertisement, disadvantaged business enterprises will be afforded full opportunity to submit bids in response to this invitation and will not be discriminated against on the grounds of religion, race, color, national origin, sex, sexual orientation, gender identity, age, disability/handicap and low income in consideration for an award.
The Board of Works and Safety, City of Warsaw, reserves the right to reject any and all bids and/or to re- advertise.
