REQUEST FOR PROPOSAL This Request for Proposal is to operate a Latch-Key Program at Whitko Community Schools Pierceton Elementary and/or South Whitley Elementary pursuant to IC 20-26-5-3. Any for profit or nonprofit organization that desires to provide services at our facilities is requested to reply. Any additional information can be found at: http://www.doe.in.gov/ois/latchkey/docs/LatchkeyProviderFormIANadditions.pdf (1) Evaluation Criteria - Scope and Sequence of the activities offered to students - Qualifications of Program Implementers - Schedule of fees charges to parents/caregivers. (2) All other evaluation criteria being equal, the cost to parents/caregivers for the program will be the determining factor (3) Evidence of financial responsibility must accompany the proposal. (4) Discussion may be conducted with responsible parties who submit proposals to help determine selection. Proposals will be accepted until close of business on September 1, 2010.
