IN THE KOSCIUSKO SUPERIOR COURT
CAUSE NO. 43D03-1102-MF-42
    COMES NOW the Plaintiff, Lake City Bank ("Lake City"), by counsel and having filed its Complaint and Affidavit in the above-entitled cause, hereby requests that the Clerk of Kosciusko Superior Court issue the Summons by publication as follows:
    1. This Summons by publication is specifically directed to Defendants, Glen A. Bauman and Rosella M. Bauman, whose last known address was and is: P.O. Box 69, Pennville, Indiana 47369 and their present address is unknown.
    2. The nature of the above-captioned cause is that on August 17, 2006, Glen A. Bauman and Rosella M. Bauman (the "Baumans") executed and delivered to Lake City an Adjustable Rate Note (the "Note") by which the Baumans promised to pay to the order of Lake City the sum of $35,200.00, together with interest. As of February 2, 2011, the outstanding principal balance of the Note was $22,610.96.
    3. To secure payment of the Note, the Baumans executed and delivered to Lake City a Mortgage upon certain real estate, said Mortgage being executed on August 17,2006. The real estate, which is the subject matter of this cause, is described as follows, to-wit:
    Lot Number 536 in Block 29 in Yogi Bear's Jellystone Park Camp-Resort, a Subdivision according to the Plat thereof recorded on May 9, 1974, in Plat Book 6, at page 107, in the Office of the Recorder of Kosciusko County, in the State of Indiana.
    ALSO:
    Lot number 77 in Block 2 in Yogi Bear's Jellystone Park Camp-Resort, a Subdivision according to the Plat thereof on May 9, 1974, in Plat Book 6, at page 107, in the Office of the Recorder of Kosciusko County, in the State of Indiana.
    Commonly known as: 1916 North 850 East #536, Pierceton, Indiana 46562 (the "Real Estate")
    4. Lake City has accelerated the entire balance due and owing on the Note and Mortgage to the Baumans' failure to make the required payments. The Baumans are also responsible for prejudgment interest as set forth in the Note and reasonable attorney's fees.
    5. The Defendants, Glen A. Bauman and Rosella M. Bauman, named herein must answer this Summons in writing, personally or by counsel, on or before thirty (30) days from the date of the third publication hereof. If said Defendants fail to do so, judgment may be rendered against them for relief demanded.
