n 9:50 a.m. Wednesday - Roy Gene Lewis, 57, of 7881 E. CR 500S, Pierceton, arrested for probation violation. Bond: $10,000.

n 12:34 p.m. Wednesday - Mindy Sue Hayes, 31, Fort Wayne, arrested for failure to appear. Bond: $500.

n 4:31 p.m. Wednesday - Douglas John Palm, 47, of 967 E. CR 450S, Warsaw, arrested for driving while intoxicated. Bond: $550.

n 4:51 p.m. Wednesday - Lawrence Tezekjian, 52, of 1305 Sunday Lane, Winona Lake, arrested for domestic battery. Bond: $500.

n 3:15 p.m. Wednesday - Intersection of Zimmer Road and Old 30, Warsaw. Pickup-SUV collision. Drivers: Michal D. Embury, 60, Columbia City; Steve A. Jones, 71, Riverwood Ranch Road, Warsaw. Damage: $1,000-$2,500. Kosciusko County Sheriff's Department investigated.

n 5:39 p.m. Wednesday - Park Avenue, 140 feet north of Main Street, Warsaw. Car hit bicycle. Driver: Robert H. List, 56, Armstrong Road, Leesburg. Cyclist Joshua Puckett, 26, Fort Wayne Street, Warsaw, refused medical treatment. Damage: Less than $1,000. WPD.

n 5:43 p.m. Wednesday - 404 S. High St., Warsaw. Van hit parked car registerd to Melissa E. Tusing, EMS D22B Lane, Syracuse; Lauren B. Richendollar, 39, Millersburg. Damage: $1,000-$2,500. WPD.
