You are notified that you have been sued in the Court above named.
The nature of the suit against you is the foreclosure of a mortgage upon the property legally described as follows:
A tract of land located in the northwest quarter of section 31, township 33 north, range 5 east, 2nd p.m., Prairie Township, Kosciusko County, Indiana, more particularly described as follows:
Beginning at a point which lies on the north section line, 1567.5 feet east of the northwest corner of said section 31; and running thence south 0 degrees 28 minutes east, along an existing fence line,1 60.23 feet; thence south 89 degrees 57 minutes east, 285.54 feet to the west line of the plat of Lakeview Park on Hoffman Lake; thence north 0 degrees 30 minutes west, along the west plat line, 159.13 feet to the north section line; thence west along the section line, 385.5 feet to the Point of Beginning and containing 1.3247 acres, including right of way for County Road 300 North.
Commonly known as: 2983 North Murphy Lane, Warsaw, IN 46582-8429
This Summons by publication is specifically directed to the following defendant(s) whose whereabouts are unknown with their last known address as follows:
Deca Financial Services, LLC as Agent of Emergency Medicine of Indiana P.C. c/o Todd Wolfe or Highest Available Executive Officer 12175 Visionary Way, Fishers, IN 46038
In addition to the above-named defendants being served by this summons there may be other defendants who have an interest in this lawsuit.
An answer or other appropriate response in writing to the Complaint must be filed either by you or your attorney with the Clerk of the Court for Kosciusko County at:
