Flood Damage Forms Available From State

While area residents continue cleanup after last week's flooding, a questionnaire to assess damages is available for residents seeking assistance.

The Indiana Department of Homeland Security is distributing a flood-damage questionnaire to different locations around the county.

The form states that it isn't an application for any assistance.

The information is the first step in attempting to get FEMA assistance for victims of the flood.

Questionnaires can be picked up at the following locations:

- Kosciusko County Justice Building, 121 N. Lake St., Warsaw, east entrance.

- Warsaw Police Department, 2191 E. Fort Wayne St., Warsaw.

- Warsaw City Hall, 302 E. Market St., Warsaw.

- Winona Lake Clerk's Office, 1310 Park Avenue, Winona Lake.

- North Webster Clerk's Office, 144 S. Main St., North Webster.

- Pierceton Clerk's Office, 105 Walnut St., Pierceton.

These questionnaires are to be completed as soon as possible and returned to the Kosciusko County Emergency Management at 121 N. Lake St., Warsaw, IN 46580 or faxed to 574-371-2611.[[In-content Ad]]
