Care Improvement Plus Debuts Its Health Plan Benefits

Care Improvement Plus, the largest chronic special needs plan in the county, recently debuted its 2012 Medicare health plan benefits for Indiana.
One of the select Indiana counties Care Improvement Plus is in is Kosciusko.
Wabash is also one of the 10 counties.
The expansion into Indiana means around 119,000 Medicare beneficiaries will be able to join Care Improvement Plus.
Open enrollment began Oct. 15 and runs through Wednesday.
The focus of Care Improvement Plus is chronic conditions like diabetes and heart failure.
Around 44,000 Hoosiers are diagnosed with either diabetes, heart failure or both.
Lauren Montrastelle is the community relations manager for the company in Illinois and Indiana.
She said that the company’s charge is to provide quality care while also getting health care costs under control.
Montrastelle said the company has been around for about 12 years, mostly in the south and the Sun Belt.
The company’s main focuses, heart failure and diabetes, affect Hoosiers as much as anyone.
Cardiovascular deaths are the top cause of death in Indiana while diabetes is seventh.
Care Improvement Plus specializes in home visits, care coordination, transportation and pharmacist counseling.
In terms of care coordination, it looks to solve problems like financial issues and caregiver burnout.
It also deals with medication issues.
Wrapped in that are pharmacists counseling.
That, in part, helps deal with patients who are often on a myriad of prescriptions.
Montrastelle said Care Improvement Plus members are on an average of eight to 11 medications.
The company works to cut down on cost of medications, avoid redundancies and look for cheaper, generic prescriptions when available.
Being Medicare, it deals with Congress.
With more health care reforms on the way, Montrastelle said Care Improvement Plus is well-positioned for those changes.
She said the government is looking at access, quality and cost and that’s what Care Improvement Plus is looking to improve upon.
“It’s a proven model of care,” said Montrastelle.
She stated that their members, as a whole, see a 16 percent drop in hospital readmission.
There is also a dual eligible plan for couples where one person has a chronic condition but the other doesn’t.
There is also an open access plan where as long as a member’s doctor or dentist accepts Medicare, a member can stay with his or her own doctor.
The plan is also designed to work with local agencies, like senior centers and councils on aging, rather than against them, said Montrastelle.
It’s also designed to provide help for caregivers who have a loved one dealing with a chronic condition.[[In-content Ad]]Caregiver Guide Journals are given out by the company to help with medication times, advice and schedules.
Care Improvement Plus holds events and meetings to educate as well as provide support to caregivers.
