BOW Approves Change Order, '07 Contracts

The city of Warsaw has agreed to pay $4,400 for a change order to cover labor costs for concrete flat work being completed on the city's streetscape project on Market Street.

The Board of Works approved the change order payment at its meeting Friday.

Reggie Smith, project manager for Phend & Brown Inc., Milford, attended the meeting and requested an additional $8,800 payment from the city, but Mayor Ernie Wiggins recommended the city pay half.

Wiggins said the $8,800 requested payment was an increase in additional cost already agreed to for layout of the curb, winter concrete material and insulated blanket.

The change order will cover the additional labor costs for detail forming work to block out decorative bricks and planters on the streetscape project. The company did not include the amount of forming and the detailed type of forming required for the brick blockouts in the original concrete price for flat work.

Smith said the company has placed 122 cubic yards of concrete since Dec. 10. There is approximately 7,042 square feet of remaining concrete to pour.

He said the company projects its total overrun of labor hours will be approximately 161 hours at $54.67 per hour.

The board also:

• Approved a $1,855 contract between the wastewater treatment utility facility and Cottage Watchman Security Systems for providing panic button and monitoring for use by plant operators when working alone at the facility's second plant.

• Approved a $2,895 contract between the wastewater treatment utility facility and Gasvoda & Associates Inc. for a wireless repeater to improve communication with lift stations.

• Approved a $24,000 contract between the wastewater treatment utility facility and Jones & Henry Engineers Ltd. for the engineer to provide assistance with the facility's requirements, including a facility report, staffing review and sludge management review.

• Approved a $290,000 quote for the wastewater treatment utility facility for the purchase of a Vactor Sewer Cleaner.

• Approved a 2007 wastewater treatment utility budget guideline totaling $6,506,134.

• Approved an irrevocable letter of credit for $15,000 to Superior Developing LLC regarding the Eagle Crest Subdivision.

• Approved a 2007 architectural design services rate schedule for Scearce Rudisel Architects Inc.

• Approved a standard hourly rate schedule for 2007 for JFNew.

• Approved a $735 contract for 2007 between the Warsaw Fire Department and ACS Firehouse Solutions for maintenance, technical and operations assistance for 2007. The cost is an increase of $35 from 2006.

• Approved a $1,100 contract agreement for 2007 between the Warsaw Fire Department and Pro Air Inc. for maintenance and service of air compressors. The contract is a reduction of $80 from 2006.

• Approved a contract for 2007 between the Warsaw Volunteer Fire Department and Warsaw for the city to pay the department $25,000 in 2007 as reimbursement for expenses and contributions to the clothing and automobile allowance.

• Approved a contract between Kosciusko County and the city for 911 services to the county.

• Approved an agreement between the county and Warsaw for a Dodge charger vehicle to be owned by the city, and the Warsaw Police Department to use the vehicle for drug interdiction.

• Approved an $8,000 contract for 2007 between Warsaw and the Cardinal Center Inc. for the Sus Amigos program.

• Approved an $18,500 contract for 2007 between the city and Kosciusko Area Bus Service for transportation services.

• Approved a $5,000 contract between the city and KEMRAD to provide traffic control for 2007 for home football and basketball games, parades and other events.

• Approved a contract for 2007 not to exceed $24,746.40 between the city and Fred Heady who was employed as a Warsaw police officer and was injured in the line of duty.

• Approved a $8,500 lease agreement for 2007 between the city and H. Anthony Miller Jr LLC, for the amount to be paid by the company to the city. The company will cut and remove all trees and shrubs and pave and asphalt land for use by an automobile or motor vehicle parking lot.

• Approved a $3,000 contract for 2007 between the city and Vision It Now Inc. for the company to update and reorganize the city Web site.

• Approved an application for an unpaid leave of absence by Sarah LeCount of the Building and Planning Department to attend a mission event in Spain and two weeks of vacation.

• Approved 14 safety committee members for 2007.

• Approved a $12,449.59 payment to Everest Excavating LLC for construction services for Fairlane Drive.

• Approved a $33,250 payment to Clounie Landscaping Inc. for Market Street improvements.

• Approved a 2007 amendment to the 2002 sanitary sewage treatment agreement where Winona Lake will pay Warsaw $3.47 per thousand gallon for sewage services.

• Approved a new job description for the sewer collection manager to change from a department head status to sewage payment office supervisor.

• Approved a 2007 city of Warsaw Benefit Trust Summary Plan with Lutheran Preferred.

• Approved a 2007 occupational health services agreement for Warsaw with MedStat Urgent Care and Occupational Health.

• Approved COBRA rates for Warsaw for 2007.

• Approved a 2007 Warsaw Volunteer Fire Department agreement that requires volunteer firefighters to participate in training approved by the Warsaw Fire Department training officer, fire chief and captain of the Warsaw Volunteer Fire Department.

• Approved two appraisals for property at 420 Little League Drive the Warsaw Parks Department is looking to purchase for its activities and programs. The appraisals include Independent Appraisal Service at $80 per hour for 5 to 8 hours, and Paul Reith Appraisal Service at $500 within 30 days of acceptance. Two appraisals are needed for the property.

• Approved an agreement to proceed with litigation against the city's liability insurer Governmental Interinsurance Exhange, for defense costs incurred by the city in responding to IDEM's claims concerning the wastewaster treatment plant and Walnut Creek. [[In-content Ad]]
