Sally L. Baker, 71, Milford, passed away at 12:40 a.m. Dec. 14, 2016, at Goshen Hospital. 
She was born May 26, 1945, in Warsaw, to Ralph E. and Gladys Marie Hollar Berkeypile. 
She was a lifetime Milford area resident and graduated in 1963 from Milford High School. She was married May 27, 1964, at St. Mark’s Church in Goshen, to Dean E. Baker, who survives. 
She formerly was employed with United Telephone in Warsaw, was an assistant to Dr. Leonard Brooks in Warsaw, and was the athletic secretary at Wawasee High School in Syracuse for 16 years.
She formerly attended the United Methodist Church, Milford, and was a life member of the former American Legion Auxiliary, Milford. 
She is survived by her husband, Dean Baker, Milford; a daughter, Lori Chalk, New Paris; a son, Scott (spouse Christine) Baker, Warsaw; four grandchildren: Heather Chalk, Taylor Chalk, Lindsay Baker and Luke Baker; three great-grandchildren: Lilyana and Beaudyn Tucker and Chayce Chalk; and a  brother, Charles Berkeypile, Milford.
She was preceded by her parents. 
Private family memorial services are planned, and burial will be in Milford Cemetery. 
Cremation Services of Kosciusko County, Warsaw, is in charge of arrangements.
