• 1:45 p.m. Tuesday - David Gabriel Rodriguez Jr., 19, of 1032 Briar Ridge Circle, Warsaw, arrested on two charges of misdemeanor failure to appear. No bond set.
• 4 p.m. Tuesday - William A. Music Jr., 52, of 3699 N. CR 175E, #108, Warsaw, arrested for operating a motor vehicle while intoxicated with a blood alcohol content of 0.15 or more. Bond: $700.
• 5:30 p.m. Wednesday - Angela Marie Frost, 51, of 3635 E. Pierceton Road, Warsaw, arrested for theft. Bond: $600.
• 1:03 p.m. Thursday - Dominique Demond Richardson, 33, of 620 N. Zimmer Road, Warsaw, arrested for felony probation violation. No bond set.

Accident
• 3:19 a.m. Tuesday - South Ind. 15, 200 feet north of East Winona Avenue, Warsaw. Two-car collision. Drivers: Prescilla D. Irmao, 37, Wasaga Beach, Ontario, Canada; a 15-year-old Warsaw juvenile. Injury: Irmao was pronounced dead at the scene. Damage: $25,000-$50,000. Warsaw Police Department investigated.

• 8:54 p.m. Tuesday - Intersection of Ind. 15 and U.S. 30, Warsaw. Two-car collision. Drivers: Breanna R. Brown, 21, West Woodland Drive, Warsaw; Fletcher G. Pohl, 17, EMS T7B Lane, Leesburg. Damage: $2,500-$5,000. WPD.
• 1:19 p.m. Thursday - 2501 Walton Blvd., Warsaw. SUV-car collision. Drivers: Cathy S. Moody, 72, Stoughton, Wis.; Ashton W. Reed, 20, North Monroe Street, Sidney. Damage: $1,000-$2,500. WPD.
• 3:07 p.m. Thursday - 1000 Husky Trail, Warsaw. Unknown vehicle hit parked vehicle registered to Mary E. Kisser, East Backwater Road, North Webster. Damage: Under $1,000. WPD.
• 6:02 p.m. Thursday - 2850 Frontier Drive, Warsaw. Car hit light pole. Driver: Devin A. Ellis, 22, Hampton, Va. Damage: $10,000-$25,000. WPD.

• 2:18 p.m. Thursday - A theft was reported in the 300 block of South Huntington Street, Syracuse. Syracuse Police Department investigated.
• 3:02 p.m. Thursday - Jewelry was reported stolen in the 3100 block of Deerfield Path, Warsaw. WPD.
• 3:57 p.m. Thursday - Interfering with the reporting of a crime was reported in the 800 block of East Smith Street, Warsaw. WPD.
• 7:33 p.m. Thursday - Jewelry, tools, a motorcycle, a computer, three bags, and two firearms were reported stolen in the 400 block of South Union Street, Warsaw. WPD.
• 8:35 p.m. Thursday - An emergency detention order was requested in the 800 block of North Harrison Street, Syracuse. SPD.
• 12:31 a.m. Friday - A traffic stop was initiated for a person in possession of marijuana/hash oil/hashish in the 1200 block of Lake City Highway, Warsaw. WPD.
