    To Respondent; Brandon Lee Eigsti, 515 Haekelman Street, Warsaw, Indiana 46580.

    Brandon Lee Eigsti, is notified that a Petition for Adoption of a child, named Coltyn David Tracy, born to Chelsie Leigh Pendleton, on May 29, 2007, was filed in the office of the Clerk of Grant County Superior Court 2, located at 101 East 4th Street, in Marion, Indiana. The Petition for Adoption alleges that the consent to adoption of Coltyn David Tracy is not required pursuant to I.C. 31-19-4.5-3, due to the fact that Respondent, Brandon Lee Eigsti, has not had a meaningful relationship and/r contact with the child for over one (1) year prior to the date of filing of said Petition of Adoption of said child.

    If Brandon lee Eisti, seeks to contest the adoption of the child, Coltyn David Tracy, then Brandon Lee Eigsti, must file a motion to contest the adoption in accordance with I.C. 31-19-10-1 in the above named court not later than thirty (30) days after the date of service of this notice.

    If Brandon Lee Eigsti, does not file a motion to contest the adoption within (30) days after service of this notice the above named court will hear and determine the Petition for Adoption. The consent to adoption of Coltyn David Tracy will be irrevocably implied and Brandon Lee Eigsti, will lose the right to contest either the adoption or the validity of Brandon Lee Eigsti's implied consent to the adoption.

    No statement made to Brandon Lee Eigsti, relieves Brandon Lee Eigsti of his obligations under this notice.

    This notice complies with I.C. 31-15-19-4.5-3, but does not exhaustively set forth a person's legal obligations under the Indiana Adoption Statutes. A person being served with this notice should consult the Indiana Adoption Statues.

    The following manner of service of this Notice of Adoption is hereby designated; U.S. Certified Mail.
