Parkview Health Systems, Inc., 11109 Parkview Plaza Drive, Fort Wayne, IN 46845 is submitting a Notice of Intent to the Indiana Department of Environmental Management of our intent to comply with the requirements of Construction Stormwater General Permit (CSGP) to discharge storm water from construction activities associated with Parkview Kosciusko Hospital South Parking Area located at 1355 Mariners Drive, Warsaw, IN 46582.
