NOTICE OF ADOPTION NOTICE TO NAMED MOTHER

IN RE: THE ADOPTION OF OLIVIA MICHAEL CAMACHO

MICHAEL CAMACHO and MICHELE LYN CAMACHO, Petitioner(s)

vs

EMILY HECKAMAN, Respondent

IN THE KOSCIUSKO COUNTY COURTS

CAUSE NO: 43C01-2204-AD-000014

    Emily Heckaman is notified that a Petition for the Adoption of a Minor Child, namely, Olivia M. Camacho, born to Emily Heckaman and Michael Camacho on September 24th, 2012 has been filed in the office of the Clerk of the Court in Kosciusko County, located at 121 North Lake Street, Warsaw, Indiana 46580. The Petition for Adoption alleges that consent to adoption of Gracie C. Camacho is not required because Respondent, Emily Heckaman, has failed without justifiable cause to communicate significantly with the child when able to do so and has knowingly failed to provide for the care and support of the child when able to do so as required by law or judicial decree.

    If Emily Heckaman, or any other individual, seeks to contest the adoption of the child, they must file a motion contesting the adoption in accordance with Indiana Code 31-19-10-1 in the above-named court not later than thirty (30) days after the date of service of this Notice.

    If Emily Heckaman, or any other individual, does not file a motion to contest the adoption within thirty (30) days after service of this Notice, the above-named court will hear and detemIine the petition for adoption. The consent to adoption by Emily Heckaman may be irrevocably implied and she will lose her right to contest either the adoption or the validity of her implied consent to the adoption.

No statement made to Emily Heckaman relieves her obligations under this Notice.

    This Notice complies with Indiana Code 31-19-4.5-3 and Indiana Code 31-19-4.5-5 but does not exhaustively set forth Emily Heckaman's legal obligations under the Indiana Adoption Statutes. A person being served with this Notice should consult the Indiana Adoption Statutes.
