Notice is hereby given, that the Town of Silver Lake, Indiana, by and through its Council referred to as the Owner will receive bids for project: 2021 CCMG Resurfacing Design Town of Silver Lake.
Sealed Bids will be received by the Town of Silver Lake in the 604 N. Jefferson St, P.O Box 159, Silver Lake, IN 46892 until 7:00 p.m. local time, on Wednesday June 2nd, 2021, at which time they will be publicly opened and read aloud.
Project Descriptions are as follows:
1. Transition mill/butt-joints at each end of each street segment, edge grass, mill entire roadway segment, sweep clean, tack coat, wedge and level as needed, patch asphalt or base as needed, adjust castings to grade, as needed, and pave with approx. 1.5 inches of Hot Mix Asphalt and replace pavement markings in the INDOT R/W as needed at the following locations:
Herendeen St, from Main St to Town Limits
Dixie Dr, from Roosevelt St to SR15
Cemetery Rd, from Winona St to SR15
Elm St, from Cemetery Rd to North St
Neher Rd, from Neer St to Town Limits
Pleasant St, from Eldora Dr to Neher Rd
Eldora Dr, from Pleasant St to Dead End
Neer St, from SR14 to Neher Rd
Pleasant St, from Eldora Dr to Amelia Ave
Lotz Ln, from Myrtle Glen Rd to Dead End
Courtland Dr, from Edgewater St to Pleasant St
Hilltop Ln, from School Ave to Dead End
High St, from Walnut St to Town Limits
Columbia St (N), from Walnut St to Town Limits
Columbia St (S), from Town Limits to SR14
Wells St, from SR14 to Poplar St
2. Apply crack seal to all visible crack the full length from edge to edge of pavement sections as needed and allow to dry before allowing traffic at the following locations:

3. Apply rejuvenating agent full length from edge to edge of pavement sections as needed and allow to dry before allowing traffic at the following locations:

Kermit St, from Edgewater St to Dead End
Wilson Ave, from Edgewater S to Roosevelt St

Bids shall be received for a single prime Contract. Bids shall be on a unit price basis as indicated on the Bid Form.
The Owner reserves the right to reject any or all proposals, and to waive any informalities in bidding. Any proposal received after the time and date specified shall not be considered.
The Issuing Office for the Bidding Documents is Fleis & VandenBrink Engineering, Inc. (F&V): 5331 South Bend Drive, Fort Wayne, Indiana 46804: Telephone (260) 435-1414.
Bids will be accepted only from Plan Holders of Record. In order to be a Plan Holder of Record Bidding Documents must be obtained from the Issuing Office.
Bidding Documents are available to be emailed in portable document format (PDF) or via an FTP Bid Room at www.fveng.com at no charge. Alternatively, printed Bidding Documents may be obtained for a non-refundable payment of $150.00, plus a non-refundable payment of $50.00 for mailing. Contact Jay Wyss at [email protected].
Each Bidder is responsible for inspecting all streets and for reading and being thoroughly familiar with the Contract Documents and Specifications. The failure or omission of any Bidder to do any of the foregoing shall in no way relieve any Bidder from any obligation with respect to its Bid.
The Town of Silver Lake reserves the right to accept any bid, reject any or all bids if the evidence submitted by, or investigation of, such Bidder fails to satisfy the Owner that such Bidder is properly qualified to carry out the obligations of the Agreement and to complete the work contemplated therein, or to waive informalities and make the award in any manner deemed in the best interest of the Town of Silver Lake.
