Phyllis K. Smith, 82, of Warsaw, passed away peacefully, Saturday, June 26, 2021, at Parkview Hospital in Fort Wayne.

Phyllis was born Dec. 9, 1938, in Cincinnati, Ohio, to John “Jack” Nicholas and Thea McKee Scheid. Phyllis was married to Clarence E. Milburn for 35 years, raising three children and then married Timmy Smith, who preceded her in death on June 10, 2018.

Phyllis is survived by three children: Lyle Milburn, Roxana (Danny) Reffitt and Frances (Jack) Worth, all of Warsaw; four grandchildren: April (Darin) Tudor, Krystal Milburn, Cheryl Milburn and Sara Worth; also 10 great-grandchildren. Also surviving are four siblings: Joe (Barb) Scheid, of Lawrenceburg; Regina Conley, of Cincinnati, Ohio; Beverly (Bill) Frederick, of Ludlow, Ky.; and Tom Scheid, of Cincinnati, Ohio.

Phyllis was preceded in death by her parents; husband, Timmy; granddaughter, Tasha Milburn; and a brother, Chuck Scheid.

In keeping with her wishes, cremation will take place.

Heritage Funeral and Cremation Services in Warsaw was entrusted with her care.
