TO: Amber L. Burnworth
Amber L. Burnworth, who is the natural mother of Ricky B. and Keith D. Grindle, born on March 8, 2016 and February 4, 2017, or who claims to be the mother, IS HEREBY NOTIFIED that a Petition for Adoption of each child was filed in the office of the Clerk of Kosciusko Circuit Court, 121 North Lake Street, Warsaw, Indiana 46580. The Petition for Adoption alleges that the consent to adoption of Amber L. Burnworth is not required because she has not had any substantial contact with Ricky B. or Keith D. Grindle for a period of more than one year, and has not contributed to the support of Ricky B. and Keith D. Grindle for a period of more than one year.
If Amber L. Burnworth wishes to contest to the adoptions of the children, she must file a Motion to Contest the adoptions in accordance with I.C. §31-19-10-1 in the above-named Court not later than thirty (30) days after the date of service of this Notice.
If Amber L. Burnworth does not file a Motion to Contest the adoptions within thirty (30) days after service of this notice, the above-named Court will hear and determine the Petition for Adoption. Her consent will be irrevocably implied, and she will lose her right to contest either or the adoptions or the validity of her implied consent to the adoptions.
Nothing Amber L. Burnworth or anyone else may say to Ricky B. and Keith D. Grindle relieves her of her obligation under this Notice.
This Notice complies with I.C. §31-19-4-5. A person being served with this Notice should consult the Indiana adoption statutes.
