NOTICE OF ADOPTION

STATE OF INDIANA

SS

KOSCIUSKO COUNTY

IN RE: THE MATTER OF THE ADOPTION OF

O.L., a minor child

MELINDA WITMER, and JASON WITMER, Petitioners

vs.

BRANDON LONG, and WENDY SICILIANO, Respondents

IN THE KOSCIUSKO CIRCUIT COURT

CAUSE NO. 43C01-1910-AD-32

Brandon Long and Wendy Siciliano are notified that a Petition for Adoption of a child, named O.L. born to Brandon Long and Wendy Siciliano on November 13, 2015 was filed in the office of the clerk of the Kosciusko County Circuit Court located at 121 North Lake Street, Warsaw, Indiana 46580 on October 21, 2020. The petition for adoption alleges that the consent to the adoption of O.L. is not required because both Brandon Long and Wendy Siciliano, Jr. has fail to have any contact or provide for the support of the child herein for the past 5 years.

If either Brandon Long and Wendy Siciliano seek to contest the adoption of the child, Brandon Long and Wendy Siciliano must file a motion to contest the adoption in accordinance with IC 31-19-10-1 in the above named court not later than thirty (30) days after the date of the service of this notice. If Brandon Long and/or Wendy Siciliano does not file a motion to contest the adoption within thirty (30) days after service of this notice, the above-named court will hear and determine the petition for adoption. The consent to the adoption of O.L. will be irrevocably implied and Brandon Long and Wendy Siciliano will lose the right to contest either the adoption or the validity of the implied consent to the adoption.

No oral statements that made have been made to Jackalyn Roberts or Devon Henson relieve either of them of their obligation under this notice.

This notice complies with IC 31-19-4.5-3, but does not exhaustively set forth a person's legal obligation under the Indiana adoption statutes. A person being serviced with this notice should consult an attorney or the Indiana Adoption statues.
