A. Notice is hereby given that the Board of Public Works and Safety, City of Warsaw, Indiana, hereinafter referred to as the OWNER, will receive sealed bids for the following project:
Buffalo Street Plaza
B. Proposals may be forwarded individually by registered mail or delivered in person, addressed to the City of Warsaw, Clerk Treasurer. Clerk's Office, Board of Public Works and Safety, City Hall 102 South Buffalo Street, Warsaw, IN 46580, prior to 10:30 a.m. November 15, 2019 (EST). Bids received after the 10:30 a.m. (EST) deadline will not be considered, and will be returned to the bidder unopened.
C. All proposals will be opened by the OWNER at a public meeting held in the Warsaw City Hall Council Chambers, Warsaw, Indiana located at City Hall 102 South Buffalo Street, Warsaw, IN 46580 and read aloud immediately following Bid Receipt at 10:30 a.m., November 15, 2019 (EST).
D. The work to be performed and the proposals to be submitted shall include a bid for all general construction, labor, material, tools, equipment, permits, licenses, insurance, service costs, etc. incidental to and required for this project.
E. This project will be subject to all applicable local and State of Indiana Codes, local hiring and the equal employment opportunity programs.
F. All materials furnished and labor performed incidental to and required by the proper and satisfactory execution of the contract to be made, shall be furnished and performed in accordance with requirements from the contract documents.
G. Bidding Documents will be available for examination on or after October 14, 2019 at 102 South Buffalo Street, Warsaw, Indiana 46580. Hard Copies of Bidding Documents may be obtained on or after October 14, 2019 at Eastern Engineering, 1239 North Wells Street Fort Wayne, IN 46808. Contact: Jamie Vaughn - [email protected]. Phone: 260.426.3119. Fax: 260.426.3101. Bidders desiring printed documents shall pay for the cost of printing, shipping and handling.
H. Each individual proposal must be enclosed in a sealed envelope bearing the title of the project and the name and address of the bidder on the front of the envelope. All mailer packets shall have a separately sealed envelope inside the mailer. Bids may be submitted until the listed time and date and bear the following:

J. Each proposal must be submitted separately. The bidder shall affix identifying tabs to the following sheets of each proposal:
1. Itemized Proposal Form
2. Indiana State Board of Accounts Contractor's Bid For Public Work - Form 96 (Revised 2013)
3. Non-Collusion Affidavit
4. AIA A310 Bid Bond Form
5. AIA A305 – 1986 Contractor’s Qualification Statement
6. Receipt of Addendum (If Applicable)
K. Each individual proposal shall be accompanied by a Bid Bond and completed AIA Document A310-2010, Bid Bond. The Bid Bond shall be a certified check or acceptable bidder’s bond, made payable to the Board of Public Works and Safety, City of Warsaw, Indiana in the sum of no less than five percent (5%) of the bid amount; the check or bond will be held by the said Board of Public Works and Safety, City of Warsaw, Indiana as evidence that the bidder will, if awarded a contract, enter into the same with the OWNER upon notification from him to do so within seven (7) days of said notification. Failure to execute the contract and to furnish performance and payment bonds to the Board of Public Works and Safety, City of Warsaw, Indiana will be cause for forfeiture of the amount of money represented by the certified check, or bidder’s bond, as and for liquidated damages. Form 96, as prescribed by the Indiana State Board of Accounts, shall be properly completed, and submitted with the bid proposals. The Board of Works, at their discretion reserves the right to waive any and all informalities in the bidding. All bids submitted shall be good for 60 days from the opening of the bids.
L. A pre-bid conference will be held City Hall 102 South Buffalo Street, Warsaw, IN 46580 at 9 A.M. October 29, 2019 local time. The meeting will be held at: City Hall 102 South Buffalo Street, Warsaw, IN 46580. A site tour will be held immediately following meeting.
M. The Owner reserves the right to accept or reject any or all offers.
