IN THE KOSCIUSKO SUPERIOR COURT I

KOSCIUSKO COUNTY, INDIANA

IN THE MATTER OF THE

ADOPTION OF MADELYNN ANN BERGMAN

CAUSE NUMBER 43D01-1910-AD-000034

    Brandon Bergman, who is the parent of the child born on October 12, 2015, is notified that a petition for adoption of the child was filed in the office of the Clerk of Kosciusko County, Indiana.

    If you seek to contest the adoption of the child, you must file a motion to contest the adoption in accordance with I.C. 31-19-10-1 in the above named court, not later than thirty (30) days after the date of service of this notice.

    If you do not file such a motion, the above named court will hear and determine the petition for adoption. Your consent will be irrevocably implied and you will lose your right to contest either the adoption or the validity of your implied consent to the adoption.  

    Nothing that the prospective adoptive parents say, or anyone else says, relieves you of obligations under this notice.

    This notice complies with I.C. 31-19-4-4, but does not exhaustively set forth your legal rights and obligations under the Indiana adoption statutes.  A person being served with this notice should consult those statutes, and/or legal counsel.
